Need serious advice on college? Horrible grades in high school !?

I’ve done terrible in high school. I repeated 9th grade 3 times because I basically fell into this huge depression and would seldom come to school. I’m currently a junior/senior at my high school. I haven't had the "traditional" high school experience most students have and transferred to an alternative school my 2nd year as a 9th grader. My transcript reflects how bad my grades have been but my TAKS scores (I live in Texas) and overall intelligence contradict it. I know I’m smart and capable and if I could just get over the attendance problem I had and lack of motivation I know I could have been nearly with a 4.0 GPA – that’s just how fast I learn things. How would you recommend me approaching a college education? I have exactly one more year before I graduate and am considering a career focused either in art or medicine but am worried about getting accepted with my grades. I WILL be taking the SAT next month as well as the ACT. Guru, please, what do you recommend I do?
